Care that reaches
everyone at home.

The moments that matter most after a hospital stay don't ask whether you have insurance, speak English fluently, or have a family member nearby. The Bolden Foundation exists to make sure support reaches the people who need it most.

Our mission

Post-discharge vulnerability doesn't discriminate — but access to support does.

The Foundation funds community-based research, sponsors access programs for underserved populations, and invests in understanding how the social determinants of health shape recovery at home. Our work is grounded in Houston's communities and designed to scale.

The Bolden Foundation is a registered 501(c)(3) nonprofit operating as the philanthropic arm of Bolden, Inc.
