Our mission
Post-discharge vulnerability doesn't discriminate — but access to support does.
The Foundation funds community-based research, sponsors access programs for underserved populations, and invests in understanding how the social determinants of health shape recovery at home. Our work is grounded in Houston's communities and designed to scale.

Building a grant-funded pathway to Bolden's post-discharge coordination for patients who can't afford private care — focused on underserved and safety-net populations.
